During your complimentary assessment to determine the type and level of care needs,we will learn more about you or your loved one’s lifestyle, medical conditions,and the kind of assistance needed to help remain independently in the home.
We will meet with you and your family and jointly determine the scope ofcare and assisted living goals.We will explain fully the in-home care services available, answer any questions, and help with the important care decisions faced by the family with a loved one in need.
If it is determined that there is a match between you or your loved one and GOOD OLD DAYS – AT HOME CARE, the Nursing Director will suggest an appropriate Comprehensive Care Plan that encompasses physical, psychological and social needs.
Once accepted to receive services, Good Old Days – At Home Care will , attempt to match the best caregiver with you or your loved one, taking into account such important elements as needs and personality. Moreover, on-going follow-up supervision ensures that the care plan is well-implemented and that changing needs are met. We are mindful of family nuances, and we’ll create a care plan that meets almost anyone’s budget.
