Consumer Directed Services

AGAP Consumer Directed Services, LLC is a Consumer Directed Services Facilitation provider for individuals approved for Medicaid’s Waiver (CCC Plus, Community Living (CL), Family & Individual Support (F & IS, Early and Periodic Screening, Diagnostic and Treatment (EPSDT).  We assist those Medicaid individuals who are at risks of being placed in a nursing facility, but instead they choose to receive personal care and respite care services in the home.  Since 2013, AGAP has supported hundreds of individuals in the Waiver program!

Eligible participants in the program must have full Medicaid and be approved for a waiver.  The individual can be 65 years and older, children or adults with disabilities, nursing facility residents, or someone receiving services through one of the Waiver programs:  CCC Plus Waiver (formerly the EDCD and Technology Assisted Waivers), Community Living (CL), Family & Individual Support (F & IS), or Early and Periodic Screening, Diagnostic and Treatment (EPSDT).

In this program, the waiver participant is able to hire a personal care aide (PCA) to come into the home and assist with care. The PCA must meet the following criteria:

Basic Qualifications for a Personal Care Aide (PCA)

  • 18 years of age or older
  • Possess basic math, reading, and writing skills
  • Able to do the tasks listed on the plan of care
  • Have a valid Social Security number
  • Complete a criminal history records check
  • Be willing to be trained at your request
  • Must take and pass a TB Test.

A Personal Care Aide (PCA) assists with:

  • Grooming and hair care
  • Bathing
  • Dressing/undressing
  • Turning, changing positions, moving around, walking or wheeling
  • Basic meal preparation
  • Maintaining a safe environment
  • Escort and/or transport services
  • Observing, monitoring and reporting on a client’s condition
  • Light Housekeeping
  • Medication Administration
  • After school care
