Since 2013, AGAP Consumer Directed Services, LLC has been approved by the Virginia Department of Medical Assistance Services (DMAS) to provide Waiver services to individuals who are at risk of being placed in a nursing facility, but instead choose to stay at home and receive support in the home and community.  We provide consumer directed service facilitation for individuals approved for Virginia's Medicaid Waivers: Commonwealth Coordinated Care (CCC) Plus, Community Living (CL), Family & Individual Support (FIS), and Early and Periodic Screening, Diagnostic and Treatment (EPSDT) programs. AGAP Adult Day Care provides a safe and secure place for the elderly and persons with disabilities to go during the daytime hours.  Our desire is for our participants to benefit spiritually, socially, and physically from the programs and services in which we provide.
